Provider data management for health plans
A cardiologist leaves a group in March. Your systems find out in June.
Provider data management, for a health plan, is closing that gap: keeping credentialing, contracting, the directory and core administration in agreement about who is in the network, where, and from when. Gaine HDMP does it by reconciling, not overwriting. Each system receives only the change it needs, with the evidence for it.

Three months, with and without Gaine HDMP
Three months, without Gaine HDMP
- March 1
A cardiologist leaves a medical group. The group’s next roster simply drops her. Nobody sends a termination.
- March 15
The directory still lists her at the group’s two locations. A member books an appointment with her.
- April
Claims at one of those locations pay in network under her name, for dates after she left.
- June
A directory check finds the listing. The member’s complaint has already arrived, and the overpaid claims have to be recovered.
OutcomeEvery system acted on what it held. None of them held the termination.
The same three months, with Gaine HDMP underneath
- March 3
The roster arrives as usual. Gaine matches it against the provider master. The cardiologist is missing from a roster she was on last month, so her record is flagged, with both rosters as the evidence.
- March 4
A steward confirms she has left, effective March 1. Gaine end-dates her affiliation with the group and its two locations. Her license, her other affiliations and her history are untouched.
- March 4
Each system hears about it on its own terms. The directory receives the end date and stops listing her there. Core administration receives a change file for its own change control. Credentialing is notified.
- April
A claim for a date after March 1 at that location is flagged in the claims reconciliation report instead of passing unnoticed.
OutcomeThe termination is known once, dated correctly, and reaches every system the way that system accepts change.
Illustrative. Each system connects the way it allows, so how far each one aligns differs.
What Gaine keeps, and what it sends
- Every provider record, mastered: practitioners, groups, locations, networks, contracts, products and taxonomy, under survivorship rules your team sets.
- Effective dates on every relationship, so a departure from one location does not end the others.
- A picture of each system, field by field, so Gaine can send each one only the difference, with its evidence.
- Unmerge when a match is wrong, restoring both records and telling every system that took the merged one.
How changes reach your systems
Gaine has built direct API connectors for HealthRules Payer, Facets, QNXT, EZ-CAP and symplr. Every other system takes one of three routes:
- reconciled change files through your own change control;
- event notifications, for systems that subscribe;
- Gaine’s platform API, for applications that call it.
Each system can use a different route. Providers attest and update their information through a provider portal, and each update is reconciled with the rest of the record before it reaches the directory.

IDC MarketScape: U.S. Provider Data Management for Payers 2025–2026. Gaine named a Leader.
“Consider Gaine when payers want comprehensive, mature data management with an operational data layer that can distribute clean, harmonized data in real time to all of the organization’s systems in the way that they need to consume it.”

Questions
How does Gaine catch a termination nobody sent?
It compares each delegated roster with the previous one and with the provider master. A practitioner missing from a roster is flagged, with both rosters as the evidence.
Does Gaine terminate a provider automatically?
No. A steward confirms the end date. Gaine then end-dates only the affected affiliations and sends each system the change.
What format must delegated rosters be in?
Each group’s own format, sent by secure file transfer as it is today. Nothing changes for the group.

Network participation by date
Was this provider in that network on the date of service? Gaine masters the practitioner, location, network and contract as separate records, each with its own dates, so every system gives the same answer.
