Provider data management for health plans
Your directory is only as accurate as the systems behind it.
Members see the provider directory, and regulators check it, including provider-directory requirements under the No Surprises Act. But the directory is the last system to hear about a change.
Provider data management, for a health plan, means keeping credentialing, contracting, network management and core administration in agreement, so the directory is right because everything upstream is right. Gaine HDMP masters the provider record and sends each system only the change it needs, with the evidence.

Three sources, three clocks
- Credentialing verifies on its own cycle and is the authority for what it verifies.
- Contracting and network management decide who is in which network, at which location, from which date.
- Delegated rosters arrive monthly, in each group’s own format, often without an NPI.
The directory takes whatever reaches it first. So does core administration, which is why the same gap that misleads a member also pends a claim. Correcting the directory by hand fixes the symptom for a month.
Make it right once, upstream
- One mastered record for every practitioner, group, location, network, contract and product, under survivorship rules your team sets attribute by attribute.
- Effective dates on every relationship, so the directory shows who is in the network at that location today, not who was last month.
- Only the difference, with the evidence. The directory, credentialing and core administration each receive the change they need, by the route their owner chooses.
- Providers update their own details through a provider portal. Each update is reconciled with credentialing and contracting before it reaches the directory.
Accuracy you can show
Every listed value carries its lineage: which source supplied it, which rule chose it, and when it changed. Gaine also keeps a history of what each system held. When someone asks why a listing read the way it did on a given date, the answer comes from the record, not from memory.
Why did the directory say that?
A member complaint asks why the directory listed a practitioner at a location she had left. Gaine answers from the record:
- Location
- From the group’s February roster.
- “Accepting new patients”
- From her provider portal attestation on January 12.
- Departure
- Missing from the group’s April roster; flagged April 2; end date confirmed by a steward on April 3.
- Directory
- Received the end date on April 3; the listing showed until then.
Without that lineage, the same answer takes days of email between credentialing, network management and the directory team. With it, it takes one query, and the answer is the same in an audit eight months later.
Illustrative.
How changes reach your systems
Gaine has built direct API connectors for HealthRules Payer, Facets, QNXT, EZ-CAP and symplr. Every other system takes one of three routes:
- reconciled change files through your own change control;
- event notifications, for systems that subscribe;
- Gaine’s platform API, for applications that call it.
Each system can use a different route.
Proof
IDC MarketScape: U.S. Provider Data Management for Payers 2025–2026. Gaine named a Leader.
“Consider Gaine when payers want comprehensive, mature data management with an operational data layer that can distribute clean, harmonized data in real time to all of the organization’s systems in the way that they need to consume it.”
36%average improvement in provider network accuracy.
Figures as of September 2026.

Questions
Can Gaine show why a listing read the way it did?
Yes. Each listed value carries its source, the rule that chose it and the date it changed, and Gaine keeps a history of what each system held.
Does Gaine replace our provider directory?
No. The directory stays. Gaine provides the reconciled data behind it.
Does Gaine guarantee directory compliance?
No platform can. Gaine makes accuracy the result of a reconciled record, and gives you the evidence for how each listing was produced.

Network participation by date
Was this provider in that network on the date of service? Gaine masters the practitioner, location, network and contract as separate records, each with its own dates, so every system gives the same answer.

A practitioner leaves a group in March, and your systems find out in June. Gaine reads every roster against the provider master, flags who has left, and sends each system the end date.
