Claims data management for health plans

A claim is only as clean as the records it points to.

Claims data management, for a health plan, means every claim resolves to the right member, provider, location and contract in every system that touches it: core administration, payment integrity, finance and the data warehouse. Gaine HDMP links each claim to those mastered records, finds the duplicates, and sends each system only the change it needs, with the evidence for it.

Where claims come apart

  • The same claim arrives twice. A provider waiting on payment resubmits, with a different claim number.
  • The member changed. A new ID in one system, the old one in another, and the claim sits against neither.
  • The provider moved. The claim bills from a location the contract does not cover on that date.
  • The books disagree. Core administration, finance and the warehouse each show a different total for the same claims.

None of these is a claims-processing problem. Each one is a records problem that shows up in claims.

What Gaine does

  • Links every claim to mastered records: the member, the rendering and billing provider, the service location and the contract, each with its effective dates.
  • Finds duplicates and resubmissions across systems, so the same claim is not worked twice.
  • Keeps a picture of each system, so it knows which ones disagree about a claim, and why.
  • Sends each system only the difference, with its evidence, by the route that system already uses.
  • Feeds the warehouse one way, so analysis runs on the same reconciled records operations use.

What Gaine leaves alone

  • Claims processing stays in core administration. Gaine is not a claims system or a clearinghouse.
  • Analysis stays in your warehouse or lakehouse.
  • Payment integrity stays with your tools and team. Gaine supplies the resolved member, provider and claim records they run on.

One claim, four answers

A provider calls about a claim. Core administration shows it paid, finance shows it outstanding, the warehouse shows it denied, and payment integrity has flagged it as a possible duplicate. Each system is right about what it holds.

Gaine shows why: the claim arrived twice, once under the member’s old ID and once under the new one, and the two copies were matched to different member records. With the member resolved to one record, the two copies are recognized as one claim. Each system receives the change it needs: finance closes the outstanding item, payment integrity clears the flag, and the warehouse is corrected one way.

Illustrative.

How claims and changes move

  • Claims in: from core administration, or loaded into your data lake by your own processes and read by Gaine through a data share, or by file.
  • Changes out: Gaine has built direct API connectors for HealthRules Payer, Facets, QNXT and EZ-CAP. Other systems receive reconciled change files through your own change control, event notifications, or Gaine’s platform API.
  • Analysis: your warehouse or lakehouse receives the reconciled records one way.

Questions

What does Gaine link a claim to?

The member, the rendering and billing provider, the service location and the contract, each as it stood on the date of service.

What happens when two systems disagree about a claim?

Gaine shows which records each system matched it to and why, and sends each one only the change it needs, with the evidence.

Does Gaine change the claim itself?

No. Adjudication stays in core administration. Gaine corrects the records the claim points to.
